A multidisciplinary team of specialists from Pinar del Río has successfully saved the life of a 24-year-old woman who was diagnosed with a ruptured tubal ectopic pregnancy at the Policlínico Juan Bruno Zayas in Mantua, the westernmost municipality of Cuba. This emergency operation involved the coordination of two hospitals, multiple ambulances, and numerous healthcare professionals.

The incident took place on a Monday around noon when the Mantua clinic admitted the patient, who presented with free fluid in the intra-abdominal cavity, a condition known to be one of the leading causes of maternal mortality globally.

Immediately, the protocols of the Integrated Medical Emergency System (SIUM) were activated by the provincial health authorities, the SIUM Coordinating Desk, and the Abel Santamaría Hospital, the main hospital in the capital of Pinar del Río.

Swift Response in a Critical Situation

Within minutes, a team from the Provincial Maternal Commission was dispatched with medications, anesthetics, surgical instruments, blood, blood products, and intravenous fluids, ready to perform surgery on-site if necessary due to the distance.

Simultaneously, the Augusto César Sandino General Teaching Hospital in the municipality of Sandino was put on alert as an intermediate contingency point in case the patient could not be transported to the capital. "Two teams activated, two institutions ready, a high level of coordination. The best specialists on wheels moving to address the issue," described Reinaldo Menéndez from the Assembly of People's Power in Pinar del Río in a Facebook post detailing the operation.

As the team made its way to Mantua, clinic staff stabilized the patient while receiving real-time guidance from specialists, accompanied by political and governmental authorities from the area.

Unexpected Challenges During the Transport

Once the patient was stabilized, a decision was made to evacuate her to Abel Santamaría. During the transport, the ambulance experienced a tire puncture. According to Menéndez, "in seconds, the professors and crew turned into mechanics, supported by the solidarity present, allowing them to change the tire with the speed seen in Formula 1 races."

Photos shared online show healthcare personnel in white coats kneeling beside a Pinar del Río Public Health ambulance on a rural roadside, addressing the issue without halting the operation.

Upon arriving at Abel Santamaría Hospital, a reinforcement team was on standby. Surgery confirmed the initial diagnosis: a ruptured tubal ectopic pregnancy with significant blood loss in the cavity. The young woman survived and was in recovery at the time of the report.

The Larger Context of Healthcare in Pinar del Río

This case is particularly significant given the current healthcare challenges in the province. As of February 2026, Pinar del Río's health system has been operating under severe restrictions due to power outages and shortages of supplies, although emergency care remains an absolute priority.

Cuban studies indicate that between 2005 and 2018, ectopic pregnancies accounted for 12.5% of maternal deaths due to direct obstetric causes in Cuba, with 65.1% of the cases being ruptured at the time of diagnosis.

"The young woman was saved, she is now recovering, and everyone is satisfied because what was heroism transformed into a life snatched back from death," Menéndez concluded his account.
